Subject:	Request for Annual Financial Report (PDE-2057) year ending June 30, 2011 and estimated salaries and wages information for 2012-2013 due as soon as possible but no later than November 30, 2011.

Please mail or submit in PDF format a copy of your Annual Financial Report (PDE-2057 or PDE-2056) for year ending June 30, 2011, ASAP. We extract information relative to an annual financial report that must be filed with the Pennsylvania Bureau of Workers' compensation on a timely basis. Also, fiscal year 2010-2011 gross annual salary information is needed for the 2012-2013 rate computation and payroll audit report.

Please see the Estimated Salaries and Wage form to be completed and return as soon as possible.

According to the SDIC bylaws, failure to submit the Annual Financial Report (AFR) and wage requirements by the due date (December 31, 2011), shall result in penalty of 1% of the premium for the current year of coverage.
